I placed a phone order, now what I do?

 If you placed a phone order on our website, payment must still be received before the order can be sent.  If you wish to fax your payment information, you can fax it to 480-422-7350.  We need the following information:

  • credit card number

  • expiration

  • CVV number (only for American Express, Visa, and MasterCard)

  • order number and name
